Eligibility Criteria for PACE
There are four main criteria to qualify for PACE:
- Be 55 years of age or older
- Be able to live safely in the community with PACE help.
- Live within the service area of PACE of Southwest Michigan
We Serve the Following Communities:
Bangor, Baroda, Berrien Center, Berrien Springs, Benton Harbor, Bridgman, Buchanan, Cassopolis, Coloma, Covert, Decatur, Dowagiac, Eau Claire, Edwardsburg, Galien, Grand Junction, Harbert, Hartford, Lakeside, Lawrence, Lawton, Marcellus, New Buffalo, New Troy, Niles, Paw Paw, Sawyer, Sodus, South Haven, Stevensville, St. Joseph, Three Oaks, Watervliet, Union Pier
- Qualify for nursing home level of care (see definitions below)
SOME COMMON CONDITIONS THAT QUALIFY FOR NURSING HOME LEVEL OF CARE ARE:

ASSISTANCE WITH MOVEMENT
Assistance 3+ times per week with standing up, transferring from bed to chair, positioning in bed, consistent use of a walker or wheelchair.

OXYGEN
On daily oxygen, shortness of breath with daily activities.

ASSISTANCE WITH TOILETING
Adjusting clothing, cleansing, changing adult diapers, catheter or ostomy care, transferring on and off toilet.

MEMORY
Dementia, Alzheimer’s, or other memory issues that affect daily routine.

DAILY ACTIVITIES
Assistance at home with daily chores or routine activities.

NUTRITIONAL INADEQUACY
Assistance with feeding, providing oral or enteral supplements, needs home delivered meals due to inability to cook for self.

ONGOING CARE & TREATMENT
Need for ongoing physical, occupational, or speech therapy, dialysis, trach tube, end stage care.
