      <image:title>Enrollment Process - Step 1: Contact PACE</image:title>
      <image:caption>When you call or come to our center, we will explain our program and services; eligibility requirements; and payment options for PACE services. We will ask some questions about the challenges you or your loved one face and help you determine if PACE is the right fit for you or your loved one. We will then schedule an in-home visit with an enrollment specialist.</image:caption>

      <image:title>Enrollment Process - Step 2: Home Visit</image:title>
      <image:caption>Our Intake Coordinator(s) will meet with you and your loved one in your home to further discuss your needs. We will complete some assessments and apply for Medicaid for you, if needed. We will then schedule additional assessments with other members of our care team such as physicians, therapists, and others.</image:caption>
    </image:image>
    <image:image>
      <image:loc>https://images.squarespace-cdn.com/content/v1/5ed66104903b262d1ab67b19/1615898736186-MFF6H9FZDUQM60EDQ2MG/unsplash-image-Oalh2MojUuk.jpg</image:loc>
      <image:title>Enrollment Process - Step 3: Team Discussion</image:title>
      <image:caption>Next, the interdisciplinary team--made up of the healthcare professionals at PACE--share their assessments with each other and create a care plan addressing your needs. Your care plan is designed just for you and is optimally crafted to meet your personal needs and goals.</image:caption>

      <image:title>Enrollment Process - Step 4: Enrollment Visit</image:title>
      <image:caption>Once you have been approved for PACE, your enrollment specialist will make an appointment to come back to your home and review your care plan with you. They will then walk you through signing the enrollment papers. Once you are enrolled, PACE services will start on the first of the next month.</image:caption>

      <image:title>The PACE Difference - Interdisciplinary Teams</image:title>
      <image:caption>PACE participants are served by a comprehensive team of professionals. Participants meet with an interdisciplinary team (IDT) that includes doctors, nurses, therapists, social workers, dieticians, personal care aides, and others. Their needs are assessed and an individualized care plan is developed to respond to all of the participant’s needs. Participants continue to be assessed regularly to determine if their needs have changed and services need to be adjusted.</image:caption>

The all-inclusive care that PACE provides enables caregivers to spend more quality time with their loved ones.
