Primary Care

PACE coordinates all healthcare services. This includes primary care, specialty care, pharmacy services, hospitalizations, medical equipment, and more.

Primary Care Providers

PACE primary care providers are dedicated only to our PACE participants. They have a wealth of experience with patients over 55 and all the illnesses and conditions that qualify a participant for nursing home level of care. Our providers are able to visit participants in the comfort of their own home when appropriate.

Pharmacy Services

PACE provides most prescription medications. Medications and refills are delivered to the home or are sent home with our participants after visiting the Day Center. PACE has our own pharmacist on staff who is available to answer questions about prescriptions and help participants manage their medications.

Medical Specialists

PACE participants receive specialty physician services (other than emergency services) from PACE contracted providers. PACE providers coordinate the care from all specialty medical providers to ensure the appropriate continuum of care for each participant. PACE also provides dental and vision care; podiatry and audiology services; and medical equipment and supplies. In-network hospital care, laboratory services, and emergency services are included.
