PACE Establishes Full-Time Medical Team

 

Dec 7, 2021 – St. Joseph, MI – PACE of Southwest Michigan is proud to announce that, for the first time in several years, they have a full-time, PACE-employed medical provider team. Medical Director, Jennifer White, MD; Dietmar Grentz, MD; Sherry Simpson, NP; and Thomas McConnell, NP comprise the PACE provider team.

According to Health Care Trends, September 2021, “the US is falling short on the number of primary care physicians they need on a national level. The situation is even more serious in rural areas.”

PACE of Southwest Michigan Donates Backpacks and School Supplies to Benton Harbor Charter School

 

SEPT. 7, 2021 – St. Joseph, MI – Parents, teachers and students alike have all been excited to start the 2021/22 school year. The past year has been a challenging one financially for many families. Some families struggle trying to pay for backpacks and school supplies for their children. PACE of Southwest Michigan was very fortunate to have the opportunity to help the students who attend the Benton Harbor Charter School by providing over 35 backpacks and school supplies to students.

PACE OF SOUTHWEST MICHIGAN ANNOUNCES THAT JIM SCHLAMAN IS NEW CEO

Nov. 30, 2020 – St. Joseph, MI – PACE of Southwest Michigan is pleased to announce that Jim Schlaman has joined PACE as Chief Executive Officer. Schlaman brings more than 30 years of experience in healthcare to the position. “I am delighted to work with the PACE team to care for the community,” said Jim. “The PACE model of care is innovative as we strive to keep people within the comforts of home while providing access to quality medical, social, and holistic services designed toward wellness and prevention.”

PACE OF SOUTHWEST MICHIGAN CELEBRATES 5 YEAR ANNIVERSARY AND NATIONAL PACE MONTH

SEPT. 1, 2017 – St. Joseph, MI – PACE of Southwest Michigan (PACESWMI) is celebrating its 5-year anniversary and the many lives that have been enhanced by their services. PACE, which stands for Program of All-Inclusive Care for the Elderly, is a unique option for older adults who wish to remain in their home as they age.  PACE keeps older adults in their home by coordinating their medical and therapeutic treatment, providing in-home assistance, and by offering social interaction at their Day Center.

PACE (Program of All-Inclusive Care for the Elderly) Completes Virtual Ride on Route 66

Mar. 7, 2017 – St. Joseph, MI –Participants at PACE of Southwest Michigan got their kicks on Route 66 — while paving their personal road to improved health. The virtual trip was taken via the exercise bicycles at the PACE Day Center in St. Joseph.  

 By the time the challenge was completed, participants had “traveled” 5,134 miles, exceeding the distance of Route 66 from Illinois to California and back.  On the PACE NuStep recumbent bicycles, 1,584 exercise steps equal one mile. Over one third of the 165 participants enrolled in PACE of Southwest Michigan are physically capable of exercising on the NuStep machines.  

A New Option for Frail Older Adults in the Niles-Buchanan Area to Live at Home is Accepting Applications

Mar. 7, 2017 – St. Joseph, MI – People in the Niles-Buchanan area have a new option in long term care that allows them to remain in their own home as they age. PACE of Southwest Michigan has been recognized by Centers for Medicare & Medicaid Services (CMS) and the State of Michigan as a Program of All-Inclusive Care for the Elderly (PACE) provider. PACE is now working towards enrolling individuals in southeastern Berrien and Western Cass counties.

Dr. VanOosterhout Receives Anthony V. DeVito Award 

March 25, 2015- St. Joseph, Mich. – Dr. Keith VanOosterhout has received the Anthony V. DeVito II Memorial Award through the University of Michigan Geriatrics Center and Institute of Gerontology.

The Anthony V. DeVito Award was established to recognize outstanding service, dedication, and commitment to excellence in enhancing community-based geriatrics education in the State of Michigan.  This annual award was established in 1991 in memory of Anthony V. DeVito II who exemplified these attributes during his tenure with the U of M Geriatrics Center.
