Keeping participants safe in their home is our goal.

PACE provides homecare services, medical equipment, and coordination of medical care to help participants remain safe in their home. PACE employs home care nurses and aides and also contracts with homecare agencies in the communities we serve to meet the needs of our participants.

Care in your own home

PACE provides caregivers who help our participants with personal care, housekeeping, chores, grocery shopping, laundry, meal preparation, and medical reminders. If a PACE participant is unable to come to our clinic, the nursing team can assess participants in their home. Our nurses check blood pressure, medication compliance, conduct blood draws, and provide wound care.

Medical Equipment Management

Having the right equipment is essential to remaining safely in your home. Our Home Care Coordinators may provide medical supplies for our participants such as wheelchairs, walkers, hospital beds, oxygen equipment, personal emergency response systems, incontinence supplies, and other necessary items.
